This order form lists and describes all of the free materials available through the Safe to Sleep campaign (formerly the Back to Sleep campaign), which educates caregivers about ways to reduce the risk of Sudden Infant Death Syndrome (SIDS) and other sleep-related causes of infant death. Materials include English brochures for general, African American, American Indian/Alaska Native, and grandparent audiences; Spanish brochures for general and grandparent audiences; English and Spanish fact sheets and videos; door hangers; and materials for health care providers and community health workers.
