A multidisciplinary group of experts will critically evaluate the existing literature on implementation and effectiveness of the medical home model for children with special health care needs, specifically targeting children who have developed chronic healthcare needs as an outcome of critical illness or injury. Participants will critique existing scientific data, identify gaps in research, and identify scientific initiatives or investigations that would be helpful for addressing these research needs to benefit these children and their families.
